2011-2012 4-H Enrollment Information

To be enrolled for the 2011-2012 4-H year you must:

  1. Complete the online registration at florida.4honline.com beginning August 23, 2011
    1. If you were in 4-H last year, select "I have a profile" and log in with the parent e-mail address on your 4-H registration form from last year and with family password. If necessary create a new password.
    2. If you were not a member of 4-H last year, select "I need to set-up a profile" and proceed from there.
  2. Return 2 signed copies of the Florida Participation Form to the 4-H club or the Extension office.  (One copy for 4-H office and one copy for club records.)  Please email the 4-H office if turning your paperwork into your 4-H club leader.  You MUST print this form from the florida.4honline.com website by logging into your child's record under Member Reports-and choose Florida Participation Form.
  3. Please print form front to back to save paper and filing space.
  4. Return a signed copy of the Osceola County 4-H Parents Agreement/Active 4-H'er definition to the 4-H club or Extension office. A form for each youth should be signed by the parent and each youth.
  5. Print the appropriate pages from the 4-H Project List, indicating the project(s) youth are enrolling for the 2011-2012 4-H year.  Most project descriptions are available on the 4-H website. Note: All must complete the first page, as well as, any page(s) needed showing chosen project.  Turn in project list to club leader or 4-H office.
  6. Pay a non-refundable $10.00 fee.  Please make checks payable to Osceola County 4-H Foundation.
  7. Completed registration packet for club or 4-H office will include:  Parent Agreement, Florida 4-H Participation Form, 4-H Project List and Registration Fee.  Registration is not complete without forms, fee and online registration.
